Brain Injury Vision Symptom Survey (BIVSS)
For teens and adults after a concussion, brain injury, or stroke, or anyone with ongoing vision symptoms. Choose how often each happens.
Recent head injury? If you have any concussion danger sign, such as worsening headache, repeated vomiting, one pupil larger than the other, slurred speech, or seizures, call 911 or go to the emergency department. Don't take a survey first. (CDC)
This is Vision Development Center of NJ's version of the BIVSS. The original questionnaire was published by Laukkanen, Scheiman & Hayes (Optometry and Vision Science, 2017;94(1):43-50). The published version has 28 items rated on a 5-point scale. This 26-item, 3-choice version uses VDCNJ's simpler result rule, and the published cutoff score does not apply to it.
